PHYSICALS FOR SCHOOL SPORTS

Physicals are offered by the school district at no charge to the student and are scheduled throughout the year for each season of competition.  All completed physical cards must be submitted to the Athletic Office AT LEAST TWO DAYS PRIOR TO THE SCHEDULED EXAMS.   These will then be submitted to the school physician and the nurse.  IF THIS INFORMATION IS NOT SUBMITTED, PHYSICALS WILL NOT BE GIVEN.  IF STUDENT DOES NOT HAVE THE PHYSICAL FORM SIGNED BY A PARENT/GUARDIAN, IN ALL AREAS REQUESTED, HE/SHE WILL NOT BE EXAMINED.

You may print a physical form from the link below, or obtain a copy from a coach, in the building Principals’ Office or the Senior High Athletic Office. 

PHYSICALS MAY BE PERFORMED BY A PRIVATE PHYSICIAN HOWEVER THE SCHOOL DISTRICT PHYSICAL FORM MUST BE USED.   ATHLETES ARE NOT CLEARED FOR PARTICIPATION UNTIL THE SCHOOL PHYSICIAN HAS SIGNED OFF ON THE PHYSICAL.

FORMS TURNED IN AFTER THE PHYSICAL DATES ARE “LATE” AND WILL BE HANDLED ON AN INDIVIDUAL BASIS WHICH MAY CAUSE SEVERAL DAYS’ DELAY FOR PARTICIPATION.

 
 
If a student was unable to attend the school-provided scheduled exam dates, they must have their exam performed by a private physician at their own expense.  Students are not permitted to practice without a physical exam and the form must be submitted to the Athletic Office for school physician clearance before they are permitted to participate.  This may cause a delay in participation so parents are encouraged to make use of the school provided-physicals.
 
If an athlete had a junior class physical by the School Physician, the Athletic Health Examination/Parental Consent Form must be signed by parent/guardian and then approved by the School Physician.
 
The form below is used for all seasons of competition.  Sections (pages) 1-4 are used for the first physical for each school year, Sections (pages) 5-6 for additional seasons of competition, and Section (page) 7 is used only by the school for Wrestling Weight certification.
 
NOTE that under Section 5, SUPPLEMENTAL HEALTH HISTORY "CIPPE" refers to the PIAA Comprehensive Initial Pre-participation Physical Evalution, Sections 1-4, or the first physical form.  Section 6: PIAA COMPREHENSIVE PRE-PARTICIPATION PHYSICAL RE-EVALUATION AND RE-CERTIFICATION BY AUTHORIZED MEDICAL EXAMINER must be completed if any of the questions in the Supplemental Health History are either checked yes or circled.
NOTE: The PA State form above lists a sampling of athletic programs in Pennsylvania, not all available through the Greater Latrobe School District.  The following seasonal programs are currently offered at Latrobe.

The Greater Latrobe Student-Athlete Guidelines have been revised for 2013-2014.  The GUIDELINES signature sheet must be completed and submitted for participation, and the UPMC forms must be completed and submitted also.
